The 4Ms of Age-Friendly Care
The Age-Friendly Health System approach is built around four essential elements, known as the 4Ms:
- What Matters: Aligning care with each older adult’s specific health goals and preferences, including end-of-life care, across all care settings.
- Medication: Using necessary medications that do not interfere with What Matters, Mobility, or Mentation.
- Mentation: Preventing, identifying, and managing conditions such as dementia, depression, and delirium.
- Mobility: Supporting safe daily movement to help maintain function and independence.

Our swing bed care program offers a range of services tailored to support your recovery and meet your unique needs. Based on your specific condition and needs, we may provide the following services:
- Direct skilled nursing care
- Skilled rehabilitation, such as physical or occupational therapy
- Continuous physician monitoring
- Dietary and nutritional services
- Care coordination and discharge planning
- Teaching and training
- Management and evaluation of patient care plan
- Observation and assessment of the patient’s condition
Swing Bed Care is a specialized Medicare program available at rural Critical Access Hospitals. To qualify for the program, you must meet the following requirements.
- You must be enrolled in Medicare Part A
- You must have a minimum 3-day hospital stay
- Your care must begin within 30 days of the original hospital stay.
- You must require skilled nursing or therapy services, such as physical or occupational therapy.
The program covers up to 20 days of care in full and partially up to 100 days when needed. Additionally, your Medicare supplement may help cover any applicable deductibles.
